What Information is on the EOB?

The above sections that are marked in Blue are the contact information for Provider Inquiries, Submitting Appeals, and for Submitting Original Bills.The above section that is marked in Green reflects the area on the EOB that you will find the claim number associated with this bill.The External Bill Number, also known as "Control Number," is marked in Orange above. When placing a call to our Provider Support Center representatives, this information will allow for faster identification of the bill in our internal systems, so we can assist with your questions in a more timely manner.Checks are payable to the owner of the Tax Identification Number, based on IRS data, and as submitted on the medical bill. This is consistent with IRS regulations for the payment of medical expenses.
